In a sense, private-practice doctors are a privileged class, since the system is set up to let them make a living even in depopulated areas.
Back in the day, you often heard people say things like, “They can stay in business with just five patients a day.” I don’t know how true that is, but the tax advantages they receive are still pretty extraordinary…

Hospital-employed doctors are employees, so they generally earn salaries in line with the going rate for physicians. Doctors who run their own practices are self-employed, so their income varies from person to person.
What do you mean by “class”? I think it depends on how you look at it.
