If the age gap is over 25 years, I have her call me “Dad~.”
With a 20-year gap, I go traveling with her looking every bit like a second wife.

I’m a columnist and a bit of a flag-maker.
I say, “Oh, this is my wife, ___.”
I tell the woman beforehand, “If someone from work happens to see us out somewhere, just go along with it and read the room, okay? lol” So she’ll probably play along.
If someone says, “Oh, you have such a young wife,” I’ll just add years to her age on my own: “No, she’s older than she looks. She’s 30.”
Even if I can’t hold it in and start laughing halfway through, or they think, “There’s no way that’s true,” I still stick to the wife story.
I just laugh it off and bluff my way through.
And even if I take a different woman to the same restaurant, I’ll still say:
“Oh, this is my wife, ___.”

If they were a proper staff member, I don’t think they would ask something like that.
If they do ask, you could have the manager come over and say:
“There’s a staff member asking me personal questions. Could you tell me why they’re asking?”
Of course, you don’t necessarily have to go that far. Ignoring them is also an option, and there are probably other ways to handle it.
Oh, and be careful not to demand that they kneel and apologize. Forcing someone to prostrate themselves can potentially lead to an arrest for coercion. (Someone was arrested for that a few years ago, and it made the news.)

I’d simply say it was work-related and put an end to the conversation. After that, I’d stop going to any restaurant with staff that lacking in tact. For the record, I’ve never actually been asked that.
