With all the different individual traits people have, like dark nipples, large areolas, or large, dark labia, I’d say it’s simply one of many variations. It would be a little difficult if there were no sensation, but even people without inverted nipples can have low sensitivity or find it painful and not let you touch them. So I don’t think this is an issue limited to people with inverted nipples. “You can’t have sex during surgery” → “You can’t have sex until you’ve recovered after surgery.” You obviously shouldn’t have sex during the surgery. I seriously looked it up, and apparently it can qualify for insurance coverage if it causes problems with breastfeeding.
