What’s the point of giving your opinion when nobody asked for it? If they haven’t said anything, I think it’s better not to say anything either.
And even if they did ask, saying that you preferred how they looked before is basically the same as telling them to get more work done. Besides, they originally had surgery because there were things they didn’t like about their previous appearance.
So rather than having surgery to return to how they looked before, they’ll probably interpret what you said and move in that direction instead.
