I think it depends on how close you became. But if it was only a few times, he’d probably just think, “You look familiar,” and leave it at that.

As Kazu mentioned, it may be hard to tell while she’s wearing a mask, but I think it would be obvious if she took it off. They’re two people who share deep memories, so I would recognize them. Even if I did, though, I’d probably pretend not to notice and carry on as usual. In particular, the woman I suspected of misrepresenting her dating preferences taught me a lot about conducting an arrangement while keeping my identity confidential. Looking back, it was a valuable experience. Good or bad aside, I realized that people like that do exist in dating clubs.

It may or may not happen. It probably depends on the situation—things like how forgiving they are, whether their impression of you changes, and whether you’re wearing a mask.
