He said, “I want you to be exclusive with me, so let’s make it a monthly arrangement.” Starting next month, it’ll be 150,000 yen per month—but does that amount include an adult relationship (intimate), or would extra be added each time? I feel awkward asking, but should I just ask him directly?

We never actually agreed on the terms for that…” Of course, if you’re okay with having sex around five times a week—about 20 times a month—for ¥150,000, then there’s no need to rock the boat. But since you asked here, I assume you thought the ¥150,000 per month was for dinner only, and are now feeling that maybe it isn’t—or that you don’t want an adult relationship for ¥150,000. So I think it may be best to make it clear that what you agreed to was dinner only, and bring up yourself that an adult relationship would be a separate matter. Whether you’re genuinely comfortable with the arrangement is important. I don’t think you should feel bound by your own words just because you already said “okay.”

If you say it’s a monthly arrangement, that means the stated amount covers everything for the month, right? When you park in a monthly parking lot, do you pay an extra fee every time you park? That would be ridiculous. I think this is just a matter of wording in Japanese. Oh, and for the record, a monthly arrangement without an adult relationship might be possible. It depends on what you include in “everything.” You could also say, “You’re the only one I have a monthly arrangement with—without an adult relationship,” and technically fulfill what they’re asking for. If you’re not comfortable with just any amount, make sure you negotiate for yourself. Generally speaking, when someone says 150,000 yen a month, it’s reasonable to assume that amount includes an adult relationship (intimate). He’s asking you to be exclusive with him, so whether that amount is satisfactory is up to you.
