Seeing someone while forcing yourself to endure it is nothing but painful. If that’s how you feel, I think it would be better to end things.
That said, you don’t want to break up because you’d lose the allowance, right?
If so, I think that, as things stand, he’ll eventually be the one to bring up ending the relationship.
It’s almost a question of which person will get fed up first: the one enduring things in exchange for money, or the one paying money and wondering why he’s putting up with it.
I have a feeling the man will be the first to think, “Why am I paying for this if I have to put up with it?”
And that’s when the relationship will probably end.
If you don’t want to break up, then when he says he wants to do it twice, simply respond with a smile and say, “Sure.” I feel like that alone would make him completely smitten.
Whether you’re okay with breaking up or don’t want to break up, you should think carefully about that.
Also, for your future, try to find a papa (a male supporter) you don’t have to force yourself to endure things for. Once you meet a papa you’re compatible with, you’ll realize that you can see him without having to put up with anything.

It seems like we’d both be better off moving on to someone else.
What’s the point of having a monthly arrangement with a woman who seems like she’s just waiting for it to be over?
And then doing a second round on top of that—what’s supposed to be enjoyable about it?

Honestly, I’ve never entered into a monthly arrangement with an older man I didn’t like—I only do monthly arrangements with someone I genuinely like, including the physical chemistry—so I don’t really understand how you feel in the first place.
Did you accept because you were happy he suggested a monthly arrangement, only to realize afterward, “Actually, he creeps me out! I hate this”?
If so, wouldn’t it be better to switch to a pay-per-meeting arrangement? Being treated however a creepy guy wants just for ¥200,000 a month will only damage your self-esteem. The only way to avoid that is to take the lead and keep control of the situation. With pay-per-meeting, you won’t be treated like you’re available whenever he wants just because you receive a fixed amount.

Maybe he asks for a second round as a bit of playful banter, even when he isn’t particularly interested, because he’s usually turned down.
It depends on the man’s age and how long you’ve been seeing each other, but if you agree once in a while, I think he’ll eventually stop asking. Since the terms are fairly favorable, why not put up with it just a little longer?

I’ve never dated someone I disliked that much, so I wouldn’t know.
You can tell whether you’re compatible after one or two dates.
By “how to turn him down,” do you mean accepting the ¥200,000 but declining intimacy?
All you can really do is say you don’t want to. Whether the ¥200,000 allowance still stands is something you’ll need to discuss with your papa (a male supporter).

It sounds tough to put up with both the dinners and the intimacy.
Finding a different papa (a male supporter) would probably be best, but as a last resort, why not ask, “Would dinner only be okay? I’m just not comfortable with sex because of my constitution.” He might still be willing to give you the same 20,000 yen for dinner twice a month.
The man probably finds sex boring and unsatisfying too. In that case, couldn’t he take care of that with someone else?
