I feel truly ashamed. A doctor I know told me that my symptoms are consistent with syphilis and urged me to get tested as soon as possible.

I honestly don’t understand why you wouldn’t get tested.
Until fairly recently, syphilis in Japan could apparently only be treated with oral medication. (I read that this was because someone had died from penicillin shock in the past, though I also read that it was due to improper treatment.) Treatment apparently took a very long time, whereas overseas, it could be treated with a single injection.
However, it seems that Japan approved the injectable medication Stelize between 2021 and 2022, so now syphilis can apparently be treated with just one injection here as well.
That said, if you’re not even willing to get tested, then doing papa-katsu (compensated dating in Japan) in your current situation seems unrealistic.
If you get tested and the result is negative, you can continue with papa-katsu as usual. Even if it’s positive, it can be treated with a single injection.
Either way, I think you’ll be able to resume papa-katsu soon.
(I read that although it’s just one injection, the time until recovery is the same—about two weeks.)

If you want to do papa-katsu (compensated dating in Japan) while having syphilis, I assume you’re looking for dinner only (no intimacy), right?
If that’s the case, there are people who do dinner-only arrangements regardless of syphilis, so it should be possible.
That said, if you honestly say you want a dinner-only arrangement because you have syphilis, wouldn’t everyone be put off?
People who do dinner-only arrangements are attractive and enjoyable women even without an adult relationship (intimate).
They certainly wouldn’t be the kind of people who make a point of advertising their illness.

Please stop all sexual activity—including with your boyfriend—as well as using dating clubs, apps, or compensated dating, and focus on getting treatment immediately. Even if you don’t intend to spread it, you could still infect someone.
