Since occupations are stored only as text in the database, it isn’t possible to extract them and calculate the exact numbers. But my impression is that the ratio of employed doctors to doctors with their own practices is about 3:1.
That’s why, when a nurse from a general hospital came in to register, she was similarly worried about being recognized.
In that case, we registered her with a blurred photo so that only the blurred image appeared in the women’s listings. This eliminated the risk of anyone other than men who made an offer recognizing her.
Also, when a man was interested, he would request an unblurred photo. Normally, we would send the unblurred photo individually, but in this case, we chose not to show it if the man was an employed doctor.
The approach above can reduce the number of offers, so I can’t strongly recommend it. Still, it is an option if you would like to register that way.
Another possibility is to register at a new location. New locations don’t create a women’s listing in the first place.
So if you’re considered beautiful or get a lot of attention—or even if you’re not especially striking at first glance but men tend to become completely captivated once they date you—it might be worth registering at a new location, where there’s no need to worry about being recognized.
If you don’t know how to apply to a new location, you could ask through the Universe application page:

(This page is for women.)
Something like, “I saw in the anonymous question box that I could inquire here. Could you tell me how to apply to a new location?” should get you the information you need.
Alternatively, there’s a chat system on the website. You can switch to a live representative from there and ask your question that way.
