Just tell him, “I had my wisdom tooth removed, and it’s swollen, so I can’t meet,” and apologize. Be sure to suggest an alternative date.
If you don’t offer another date, he may think you simply don’t want to see him anymore.
If your papa (a male supporter) has made a restaurant reservation, there may be a cancellation fee starting about three days beforehand, so make sure to contact him as soon as possible.

This may be a different perspective from the other answers, but if you’re a Type C or D, there’s a good chance your partner is expecting an adult relationship (intimate) that day.
Since your mouth is more prone to infection after a tooth extraction, I think it would be best to refrain from certain activities.
In that sense, I think you should explain the situation to the club.

It really depends on how swollen your face is.
Even if you feel like your face is extremely puffy, if it looks more like “maybe a little swollen?” to the other person, it may be safest to let the club staff know but still go on the date as planned.
The reason is that rescheduling for a legitimate reason generally won’t result in a penalty (though unfortunately there may be one if it’s on the day of the date).
If you can’t make the date on the originally scheduled day, you might assume it can simply be moved to another day. However, there’s a fairly high chance—maybe 30–50%—that he’ll make an offer to another woman instead, and he may never contact you to reschedule afterward.
That said, if your face is so swollen that you look like Anpanman, then I think the disadvantages of changing the date would be outweighed by the benefits.
Either way, I think you should let the club know. When the club explains the situation to him, he may be considerate and suggest another date, such as, “How about sometime next week?”
To sum up:
1. Tell the club about the situation.
2. Let them know that, if he doesn’t mind, you’re willing to go on the date as planned.
That’s probably the best approach.

I think you should contact the club and discuss it with them.
In rare cases, the club may reach out on the person’s behalf if they’re injured or not feeling well.
