Condoms can come off sometimes. Regular ones don’t reach all the way to the base, so you need an LL size. The 0.01 mm ones are out of the question—they always come off. It’s terrifying when a condom gets left deep inside a woman’s vagina.
In the questioner’s case, since her partner pulled out and went outside, he must have at least noticed that the condom had come off. Normally, he should have put on a new one at that point. It’s a huge mood-killer, though.

When a condom “slips off,” it’s 99% deliberate, in my opinion.
They know perfectly well how small their penis is. They probably say something like, “Oops, this condom is too big—it slipped off,” while hoping to get the chance to go in bare.

If the condom was too large and slipped off, that would mean the man’s penis was considerably small. Was it really that small?
If it was only about 2–3 cm in diameter when erect, I can see a regular condom slipping off. But if it was that small, you yourself should have realized that it slipped off because of his size.
Apparently, Okamoto’s standard-size condoms have a diameter of 33 mm. Since condoms stretch, they can also be worn by men around 40 mm in diameter. However, for men under 33 mm, the condom won’t be tight enough and may slip off.
That said, the fact that 33 mm is considered the standard size suggests that most men are larger than that, so in general, condoms don’t slip off simply because of their size.
However, according to Okamoto’s research, 18% of men are 34 mm or smaller. That would mean 18% of men need to use small-size condoms.

Normally, you wouldn’t notice if the condom had slipped off, so you would assume it was still on and ejaculate while still inside. The condom is supposed to be there.

If he pulled out before ejaculating, that means he already knew the condom had come off.
And if he knew, it was probably intentional. He may have shown you that he put on a condom, then loosened it just before penetration so it would come off easily.
That would result in the condom being left inside you.
He probably wanted to do it without a condom, but since he didn’t want to risk pregnancy, he removed it and finished outside.
He may be worried about pregnancy for now, but I think he’ll eventually start ejaculating inside you.

Personally, I’ve never had a condom slip off.
Apparently, if you remain penetrated after ejaculating, the penis can become soft, and the condom may be left behind when you pull out. That’s why condom instructions say to withdraw promptly after ejaculation while holding the condom at the base of the penis with your fingers so it doesn’t come off.
It may also slip off if the penis becomes soft during sex.
A condom can also come off if it isn’t properly rolled all the way down to the base, or if its size is completely unsuitable. I’ve also heard of condoms tearing. However, they generally won’t tear unless you stretch them too much with your fingers or damage them with your fingernails when putting them on. It’s not impossible that a condom was defective and already damaged, but apparently condoms from well-known manufacturers have been inspected for tears before shipping for many decades.
When he says, “I pulled out before finishing and ejaculated outside,” does he mean that he pulled out while still wearing the condom, removed it, and then ejaculated?
