For birthdays and gifts, you should ask the woman what she wants most and give her that.
I don’t think a surprise from an older man she doesn’t even like would make her happy at all.
What women really think is probably: “If you’re going to surprise me, just give me the money.”
So Fuu takes the opposite approach and doesn’t do anything unnecessary. I don’t give birthday presents or plan surprises. I leave that to their boyfriends or generous (well-paying) papas, haha.
I simply avoid taking them out on important event days whenever possible and stick to straightforward, no-pressure papa-katsu (compensated dating in Japan).
I might arrange a small birthday celebration during a meal on a date in their birthday month, though...

I think surprises are for people who are confident the other person will be happy with them.
If you’re not confident, I think it’s better to ask them. But if you do, make sure to clearly state your budget, for example: “Is there anything you’d like that costs under ¥100,000?”
Otherwise, if they name something expensive, you’ll end up having to either buy it even though you can’t really afford it or disappoint them.

If you’re not sure, I don’t think you should make it a surprise.
1. Ask her directly.
2. Go to a shopping mall and shop together.
3. Give her a gift card (you’ll find plenty of options online).
When you don’t know each other very well, option 3 is convenient because she can use it however she likes—or even convert it to cash.

If you’re unsure, it’s best to ask her directly. But if you’re going to look for something, I think a surprise is definitely better—and more likely to make her happy.
That said, surprises won’t be appreciated if you get them wrong…

Is she the type of woman who enjoys surprises?
Or is she happier receiving cash?
That’s where it starts.
