At the moment, he isn’t seeing anyone other than me, and I caught the STI from another man and passed it on to him. I’m worried things will become awkward if he finds out I’ve been seeing someone else, and I don’t know how to tell him tactfully.

If that’s the case, the man should already understand the risks, so she should simply be honest and tell him she contracted an STI. That said, no matter how she explains it, there’s a chance the relationship may end. But since few women are willing to go without protection, he may want to resume seeing her once she’s recovered.
