If you’re referring to an offer from Universe Club, turning it down won’t lower your rating.
That said, they’ll ask you why, so I think it’s best to have a reason ready. Something safe would be:
“My parent was recently hospitalized, and things have been hectic, so I may not be able to accept offers until things settle down.”
They might then suggest hiding your profile from view, but I also think it would be fine to take a short break.
Alternatively, you could say, “I can’t travel far for the rest of the year, so please only send me offers in Tokyo or the surrounding Kanto area.” Then, for the rest of the year, the staff may automatically turn down offers from other regions, or add a note saying “Kanto area only” to your comments.
For simple requests like these, I think it’s fine to let the staff know.
The kinds of things that would lower your rating with the staff are probably things like last-minute cancellations, rude language, or taking a long time to reply.
Even if you don’t accept an offer, I don’t think your rating will go down as long as you have a reason.

Trying to make a little extra money off transportation expenses could hurt your rating through feedback from male members. Turning down an offer (a setup), though it may leave a bad impression on the staff, probably won’t reduce your star rating.

I also wouldn’t want to make an offer to someone like you, so please state in your profile that you don’t accept offers from men outside your area.

When was this? For some reason, COVID has died down and the state of emergency has been completely lifted. Isn’t this the busy season?
I believe Universe changed its system so that a woman’s rating is determined by the men she actually meets, so turning someone down before meeting them shouldn’t affect her rating.
That said, if money is your top priority, I think you should make that clear in your profile and comments. There are plenty of women who do.
In any case, please stop pretending to be interested just to get lots of offers, then meeting men and picking and choosing. It’s a nuisance, since men pay a referral fee to meet you.
(It’s fine to turn down a setup. No referral fee is charged in that case.)
