I don’t think there’s any problem at all.
Joe’s opinion is that type fraud simply isn’t possible with Dating Type B.
If you’ve exchanged LINE contacts, turn him down gently and tactfully. If he responds angrily, block him immediately. Save the messages in case the club contacts you, and if you receive a complaint from the club, use those LINE messages to defend yourself.

There are definitely times when you think, “I knew it…” You’ll be fine.
Politely tell him that you’ve become scared. But if you’re going to turn him down, do it quickly—right away.
If he starts arguing or giving you a hard time, just block and delete him. You can also casually message the staff on LINE explaining why you turned down your date with Mr. ○○. That way, if the man leaves feedback, I don’t think your rating will be affected too much.

If you’ve only been on two dates, turning him down won’t be considered a relationship-type scam.
Even if he calls it that, the staff will likely just brush it off.
The general idea is that if you make your decision by the third date—after determining whether an adult relationship (intimate) is possible—it wouldn’t be considered a scam.
However, if you’ve gone on four or more dates and then say an adult relationship isn’t possible, the staff are more likely to side with the man if he accuses you of a relationship-type scam.
That’s because if you only want dinner dates, you should choose Relationship Type A.

I think it’s fine to just be honest and write something like:
“This is my first time, and I realized an adult relationship (intimate) makes me uncomfortable. I feel bad continuing to see you if we’re only going to have dinner, so I don’t think I can continue seeing you. Thank you for treating me to dinner.”
Something along those lines should be fine.

If you’ve already had dinner twice and discussed the terms, the man is probably expecting the third meeting to go further as well.
So I feel it’s about a 50–50 chance that he’ll accuse you of a “dating-type scam.”
As for the questionnaire, you sent it after the first dinner, so it probably won’t have much impact on your rating.
It might be best to explain the situation to the club in advance, apologize, and let them know that you may receive a complaint.

Yes, I think it’s perfectly fine to turn them down that way.
I don’t think this would count as dating-type fraud, and most people probably wouldn’t see it that way either.
